New Patient Forms

To reduce your wait time and make your first visit as smooth as possible, we encourage you to complete your new patient forms before you arrive. When you schedule your first appointment, we will email and/or text you a link to fill them out online — no printing, no bringing them in. This saves us both time and lets us focus on you, not your paperwork.

Before your visit

Making Your First Visit as Easy as Possible

We are excited that you would consider honouring us with being your partners in health. At Essential Health Group, we offer a patient-centred, evidence-based approach to health, healing, and pain management. Everything we do — from answering the phone to greeting you at the door — is all about your experience.

If you feel welcomed, relaxed, and cared for, the healing process begins the moment you walk in. Your typical new patient visit may last anywhere between 40 minutes and 90 minutes depending on the provider you see. We will welcome you at the door, escort you to a private room, and offer you some water while you settle in.

Your first visit

What Can I Expect?

Visiting an integrative healthcare facility is a new experience for many people. Whether you’ve been referred by a friend, done your research online, or simply reached the point where living in pain is no longer acceptable — your first visit to Essential Health Group is designed to be thorough, unhurried, and centred entirely around you.

Step 1 — Your History & Goals

You’ll begin by completing your new patient paperwork — either online before you arrive or in a private room on the day. Upon meeting your provider, you’ll discuss your concerns, expectations, and health goals, and have the opportunity to ask any questions. This conversation is the foundation of your care plan — we listen before we act.

Step 2 — Physical Examination

We perform a thorough physical examination and, where appropriate, advanced diagnostic testing. This may include a surface EMG to assess how your muscles are functioning, an infrared paraspinal thermograph to identify inflammation along the spine, a dual inclinometer to measure your range of motion, or an acugraph to determine whether acupuncture may benefit your care.

Step 3 — Your Care Plan

Once we have reviewed the results of your examination and discussed your history, we build a care plan together. Treatment begins only when you are completely satisfied and ready to move forward. As an integrative facility, you may work with one or more of our providers concurrently — this is often the most effective way to get out of pain quickly while reducing the overall duration of treatment you need.

Not sure where to start?

Dr. Wellwood Will Help You Find the Right Care

If you’re not sure what kind of care you need, you will most likely begin with Dr. Wellwood, who acts as our resident gatekeeper. Because of his advanced and broad-ranging knowledge across physical medicine, regenerative medicine, chiropractic, and nursing, he is uniquely placed to assess your situation and direct you to the right provider or combination of providers within our team.

As an integrative facility, it is common for new patients to work with more than one provider concurrently — this coordinated approach is the most efficient way to get out of pain and back to full function.

Serving Those Who Served Us

Our psychiatric and pain management teams understand the weight that veterans and first responders carry. We’ve built our services around helping you carry less of it. Our psychiatric team is led by retired USAF Staff Sergeant Candie Wellwood, DNP, PMHNP – a veteran who has walked the same path.

We're Excited to Meet You
