Sciatica — the radiating pain that travels from the lower back through the buttock and into the leg — is one of the most disabling pain conditions we treat. Our integrated team — led by Dr. Christopher Wellwood NP-C, MSN, DC — identifies the exact cause of your sciatic nerve irritation and builds a treatment plan designed to provide lasting relief.
Understanding Sciatica
Sciatica is not a diagnosis in itself but a symptom — describing pain that follows the path of the sciatic nerve from the lower back through the hip and down the leg. The underlying cause can be a herniated disc, spinal stenosis, piriformis syndrome, sacroiliac joint dysfunction, or degenerative changes in the lumbar spine.
Getting the right diagnosis is critical to getting the right treatment — which is why our multi-disciplinary evaluation is so important. Many sciatica patients come to us having been treated for only one possible cause when the actual driver was something else.
Types of Sciatica We Treat
Our Medical Lake practice treats patients across the full range of sciatic nerve presentations including:
- Disc herniation sciatica — disc material pressing on the sciatic nerve root causing radiating leg pain, numbness, and weakness
- Spinal stenosis sciatica — narrowing of the lumbar spinal canal compressing the sciatic nerve roots
- Piriformis syndrome — piriformis muscle irritation compressing the sciatic nerve in the buttock
- Degenerative disc sciatica — disc degeneration reducing foraminal space and irritating the sciatic nerve
- Sacroiliac joint sciatica — SI joint dysfunction causing buttock and leg pain that mimics true sciatica
- Facet joint referred pain — lumbar facet joint irritation causing referred buttock and thigh pain
- Post-surgical sciatica — persistent or recurrent sciatic symptoms following lumbar surgery
Our Diagnostic and Treatment Approach
Effective sciatica treatment starts with identifying the exact source. Our integrated team uses chiropractic assessment, physical medicine evaluation, and regenerative medicine expertise to build a targeted treatment plan — not a generic one. Many of our sciatica patients see significant improvement within weeks when the correct source is identified and treated appropriately.

🦴 Chiropractic Care
Lumbar chiropractic adjustments to reduce disc pressure, improve spinal alignment, and relieve sciatic nerve compression — one of the most effective conservative treatments for disc-related sciatica.
🧪 Neural Prolotherapy
Perineural injections targeting the sciatic nerve and its branches — normalizing nerve function and providing direct relief for the neural component of sciatica.
🏃 Physical Therapy
Targeted sciatica rehabilitation including nerve mobilisation, core stabilisation, and movement retraining — reducing sciatic nerve irritation and preventing recurrence.
🌿 Prolozone Therapy
Regenerative injections for the lumbar discs, SI joint, and surrounding structures contributing to sciatic nerve irritation — addressing the structural source of sciatica.
🩸 Platelet Rich Plasma (PRP)
PRP for disc herniations and lumbar joint injuries contributing to sciatica — stimulating repair of the damaged structures compressing the sciatic nerve.
🖐 Massage Therapy
Soft tissue therapy targeting the piriformis, glutes, and lumbar musculature — reducing muscle tension and spasm contributing to sciatic nerve irritation.
💊 Medical Management
Primary care-based evaluation including diagnostic coordination, imaging referral, and medication management for sciatica.
⚡ Physical Medicine
Traction therapy, electrical stimulation, and TENS for sciatica — targeted physical medicine interventions to reduce nerve compression and manage acute sciatic pain.
Sciatica FAQs
How long does sciatica take to resolve?
Mild sciatica from muscle involvement often resolves within a few weeks with proper treatment. Disc-related sciatica may take 8–12 weeks or longer. Early treatment significantly improves outcomes — chronic, untreated sciatica becomes progressively harder to resolve.
Can chiropractic care make sciatica worse?
In the vast majority of cases, no. Chiropractic care is one of the most effective and safest treatments for disc-related and mechanical sciatica. Dr. Wellwood evaluates your specific condition before recommending any treatment and adapts the approach based on your presentation.
Do I need an MRI before treatment?
Not always. Many sciatica cases can be diagnosed and treated effectively based on clinical examination. If imaging is required, we will coordinate a referral. Having existing MRI results available at your appointment is helpful but not essential.
What if my sciatica hasn’t improved with previous treatment?
Many of our sciatica patients come to us after unsuccessful treatment elsewhere. Our multi-disciplinary approach — particularly neural prolotherapy and regenerative injections — often delivers results where single-discipline treatment has not.
Is surgery ever necessary for sciatica?
Most sciatica cases resolve with conservative and regenerative treatment. Surgery is rarely the first option and is typically only considered for severe, progressive neurological deficit or cases that have genuinely failed all conservative options.
